The practice is still open for new patients living in the centre of Amsterdam, the Jordaan, the Westerdok island and the Prinsen island to register.

When you want to register, please fill in the form below in full, print it out and bring it with you together with your insurance card to the practice.

There we can enter you into our system and welcome you as a new client.
